NOTE: e-Permit allows more than five digits to be entered in case of 'Other' state.
Select Country.
Enter applicant's alternate Phone Number in Phone 2.
Enter applicant's Fax.
Enter applicant's E-Mail.
Select type of Applicant from the drop-down menu.
If the applicant chooses to act as the agent, please check the option, "Will act as agent".
NOTE: Do not check the above option if an agent is engaged.
Click Save.
NOTE: Click Cancel to clear all details entered.
In case of more than one petitioner, add details of another applicant of the petition.
